Antimicrobial resistance in children is rising globally, threatening the efficacy of life-saving antibiotics according to a new monitoring platform. Researchers analyzing 106,000 infection samples across 82 countries found resistance increased in every region between 2004 and 2022, with projections indicating a severe decline in treatment options by 2035.
The global health community now has a clearer view of how antibiotic resistance is impacting the world’s youngest patients. A collaborative study led by the Murdoch Children’s Research Institute (MCRI), alongside the University of Sydney, the Clinton Health Access Initiative (CHAI), and The Chinese University of Hong Kong, has launched the AMR in Kids platform. This tool is designed to provide region- and pathogen-specific forecasts, filling a critical data gap for clinicians and policymakers struggling to treat common childhood infections.
Rising Resistance in Global Pediatric Care
Between 2004 and 2022, antibiotic resistance increased in every region studied. The trend is particularly concerning for babies and children in intensive care units, as well as those living in countries with limited healthcare resources. As reported by the MCRI, the rise in resistance is driven primarily by Gram-negative bacteria, which are responsible for severe, life-threatening conditions like pneumonia and sepsis.

The study, published in the journal JAMA Pediatrics, highlights that children are uniquely vulnerable. They not only experience higher rates of bacterial infection than adults but often face a significant lack of child-friendly antibiotic formulations and clear dosing guidelines.
Projected Threats to Last-Line Antibiotics by 2035
The forecasting models integrated into the AMR in Kids platform provide a sobering outlook for the next decade. By 2035, resistance to “Watch” and “Reserve” antibiotics—drugs the World Health Organization classifies as critical to preserve for when first-line treatments fail—is expected to climb substantially.
Associate Professor Penelope Bryant, an MCRI researcher, emphasized that the data reflects a clinical reality where first- and second-line drugs are increasingly failing.

Clinical Challenges and the Need for Targeted Action
The crisis is not limited to severe hospital-acquired infections. Researchers have observed resistance appearing in more common, uncomplicated illnesses, such as pediatric urinary tract infections. This shift forces physicians to rely on less common intravenous drugs.
Addressing these issues requires more than just better data.
While the new platform provides a roadmap for identifying emerging threats, the broader challenge remains the global stewardship of existing medications. As of 2021, approximately 840,000 deaths in children under five were associated with antimicrobial resistance, a figure that underscores the necessity of the current push for better surveillance and access to effective treatments.
