Maintaining normal blood pressure, avoiding type 2 diabetes, and not smoking between the ages of 45 and 65 are associated with almost 13 additional years of dementia-free life, according to a study published in The Guardian. Researchers from New York University Langone Health analyzed data from the Atherosclerosis Risk in Communities study, following thousands of participants over decades.
Midlife Health Factors Linked to Years Without Dementia
The analysis examined 12,409 people with an average age of 56 who were initially free of dementia. Participants were assessed for high blood pressure, type 2 diabetes, and smoking, and then followed for an average of 26 years. During the tracking period, 3,008 participants developed dementia and 5,238 died without developing the condition. Individuals who had all three risk factors were more than twice as likely to develop dementia and more than five times as likely to die prematurely from another cause, according to findings covered by MSN.
Breakdown of Dementia-Free Survival Years
The research quantified how avoiding vascular risk factors in middle age influences the timing of dementia onset. Participants with none of the three risk factors lived an average of 30.1 more years free of dementia. In contrast, those with varying numbers of risk factors saw fewer dementia-free years:
* Zero risk factors: 30.1 average dementia-free years * One risk factor: 26.3 average dementia-free years * Two risk factors: 21 average dementia-free years * Three risk factors: Just over 17 average dementia-free years
Overall, individuals with no midlife vascular risk factors lived an average of roughly 13 more dementia-free years compared to those burdened by all three risk factors. The study also observed demographic differences, noting that women lived longer without dementia than men regardless of their risk factor count, and white participants had more dementia-free years than black participants.

Our findings argue that people need to actively avert these factors in midlife as a strategy for preserving brain health for more than a decade,
said senior investigator Dr. Josef Coresh, director of the Optimal Aging Institute at NYU Langone. Coresh noted that framing results around “good years” gained through lifestyle changes is often easier for patients to understand than percentage risks.

Dr. Jacqui Hanley, head of research funding at Alzheimer’s Research UK, who was not involved in the work, emphasized that the study addresses a question important to the public: how long people can expect to live free from the condition. Professor Ian Maidment from Aston University added that the findings reinforce the principle that what is good for the heart is good for the brain.
Study Limitations and Prevention Guidance
Researchers noted that the study demonstrates an association rather than direct causation, as participants were measured for health factors just once before a median follow-up of 26 years. Experts advise viewing the results as an important estimate rather than a fixed prediction for every individual.

To help avert hypertension and diabetes, health professionals recommend addressing underlying contributors such as physical inactivity, poor diet, obesity, and alcohol intake. Preventive measures include adopting lower-fat, plant-based diets like the DASH diet, managing stress, ensuring adequate sleep, and following exercise recommendations from organizations such as the World Health Organization.
