Antiphospholipid syndrome (APS) is an acquired autoimmune disorder causing recurrent arterial or venous thrombosis and pregnancy morbidity.
Managing APS is a balancing act between preventing life-threatening clots and avoiding major bleeding. For years, the gold standard has been long-term anticoagulation using vitamin K antagonists, typically maintained at an international normalised ratio of 2.0–3.0. However, the therapeutic landscape is shifting toward more individualized care, moving beyond a one-size-fits-all approach to address specific patient phenotypes and high-risk profiles.
The Role of Triple-Positive Antibody Profiles
Not all APS patients carry the same risk. Risk stratification depends heavily on the intensity of the antibody profile. A triple-positive status—the concurrent presence of lupus anticoagulant (LA), anticardiolipin, and anti-β2-glycoprotein I (β2GPI) antibodies—is a critical marker that confers higher rates of recurrence. The β2GPI is the principal antigen recognised by pathogenic antiphospholipid antibodies, while the lupus anticoagulant is a functional coagulation assay detecting antibodies that prolong phospholipid-dependent clotting times.
For those facing obstetric complications, the standard of care shifts toward a combination of low-dose aspirin and heparin to improve live-birth rates. For others, the focus remains on secondary prevention of venous thromboembolism. A multicentre randomised trial compared rivaroxaban against warfarin for this purpose; while thrombin-generation parameters differed, the study found no increase in clinical thrombosis or major bleeding, suggesting direct oral anticoagulants may be a safe alternative for patients who do not have high-risk antibody profiles.
Targeting Neutrophil Extracellular Traps and Inflammation
Recent research is moving downstream of antibody binding to target the innate immune pathways that actually trigger clots. One primary culprit is the formation of neutrophil extracellular traps (NETs)—web-like chromatin structures extruded by activated neutrophils that contribute to thrombus formation.

This activates the SLAMF8/TREM1 axis, which amplifies both inflammation and thrombosis. Pharmacological inhibition of this specific methylation event reduced thromboinflammatory markers in vitro and attenuated vascular lesions in murine models.
Adjunctive Therapies and Multidisciplinary Surveillance
Because anticoagulants only address the clotting mechanism, an international task force has summarized the use of adjunctive therapies to modulate inflammation and overall cardiovascular risk. These interventions are designed to complement primary anticoagulant regimens rather than replace them.
- Anti-inflammatory and Metabolic: Use of statins, hydroxychloroquine, and vitamin D.
- Antiplatelet Agents: Employed to further reduce thrombotic risk.
- Targeted Biologics: Use of complement inhibitors and rituximab.

Ultimately, the goal is a shift toward a therapeutic paradigm that treats APS not just as a coagulation failure, but as a systemic inflammatory condition. This requires multidisciplinary surveillance and the search for novel biomarkers to refine how clinicians assess a patient’s prognosis.
