Coffee and Heart Health: Understanding the Latest Guidelines
Most adults can safely consume up to 400 mg of caffeine daily—the equivalent of approximately three to five 8-ounce cups of caffeinated coffee—without increasing their risk of cardiovascular disease, according to a scientific statement from the American Heart Association. The findings, titled Caffeine and Cardiovascular Disease,
were published on July 20, 2026, in the association’s flagship peer-reviewed journal, Circulation.
Establishing Safe Caffeine Limits
For millions of people, coffee is a fundamental part of daily life, serving as a common way to consume caffeine, which is the most widely used stimulant and psychoactive substance in the world. A volunteer writing group for the American Heart Association conducted a comprehensive review of research from recent decades to determine the impact of caffeine on cardiovascular health, including conditions such as hypertension, diabetes, heart disease, stroke, heart failure, and atrial fibrillation.
The analysis concluded that for most adults, an intake of up to 400 mg of caffeine per day is safe. Beyond safety, some observational studies have suggested that regular coffee consumption may be linked to a lower risk of cardiovascular disease, heart failure, stroke, and Type 2 diabetes. However, researchers noted that these findings require further study to confirm whether coffee directly prevents such conditions or if other factors are involved.
“Caffeine consumed in coffee is a key part of daily life for millions of people, and in our review of the most recent research, for most adults, intake of up to 400 mg of caffeine a day, the equivalent of up to five cups of caffeinated coffee per day without added sugars or fillers, is safe and does not increase cardiovascular risk,” said Gregory M. Marcus, M.D., M.A.S., FAHA. Dr. Marcus serves as the chair of the scientific statement volunteer writing group, a professor of medicine at the University of California, San Francisco School of Medicine, and the associate chief of Cardiology for Research at the University of California, San Francisco Health.
The Impact of Preparation and Ingredients
The health benefits associated with coffee consumption are specific to how the beverage is prepared. The American Heart Association emphasized that these benefits are tied to black coffee; the addition of sugar, syrup, or creamers may reduce the potential health advantages. Furthermore, the association advises against adding ingredients like whole milk or half-and-half, as they are high in saturated fat and should be limited.

Brewing methods also influence health outcomes. Unfiltered methods—specifically French press, Turkish coffee, and espresso, which is extracted by forcing hot water through finely ground beans under high pressure—leave behind components that can raise LDL, or “bad,” cholesterol. Specifically, these methods contain cafestol, which is linked to higher cholesterol levels. Conversely, paper-filtered or instant coffee had fewer adverse effects on cholesterol levels.
Caution Regarding Concentrated Caffeine
While standard coffee is considered safe for most, the American Heart Association advises caution regarding products containing highly concentrated caffeine. Energy drinks and energy shots can have significantly higher caffeine concentrations than traditional brewed coffee. While regular coffee typically contains 9.4–20.6 mg of caffeine per 30 mL, energy shots can contain 40–69 mg in the same volume.
According to the scientific statement, high doses of caffeine found in these concentrated sources may have harmful effects on the heart, including elevated blood pressure and the potential to cause arrhythmia. Dr. Marcus and his fellow authors emphasized that these concentrated sources should be avoided.
Complexity in Scientific Research
Understanding the cardiovascular effects of caffeine remains a complex task for researchers. Coffee serves as the primary source of caffeine in most studies, but it also contains numerous other substances that could influence health. Experimental research suggests that coffee contains bioactive compounds with antioxidant and anti-inflammatory properties, which may contribute to its potential benefits. It remains unclear whether the observed health benefits stem directly from caffeine or from these other compounds.
The authors of the statement noted that more research is needed to understand how different forms of caffeine, such as those found in tea, impact cardiovascular health. Because the current evidence is based on observational data, the association continues to study the nuances of how caffeine interacts with the human cardiovascular system.
