A new register study of over two million Swedish children born between 2003 and 2021 shows a concerning trend: more parents are declining the standard Vitamin K injection for newborns. This refusal more than doubles the risk of serious bleeding, including life-threatening intracranial hemorrhages, during the child’s first six months. The study, published in the journal Jama Pediatrics, provides the first Swedish mapping of how common it is for parents to decline the injection.

Rising Refusal Rates in Sweden

The routine administration of Vitamin K via an injection in the thigh has been a standard medical practice for newborns in Sweden and many other nations since the 1960s. The vitamin is essential for blood clotting, preventing rare but severe internal bleeding that can occur in infants who cannot yet produce the nutrient themselves. Because a child with Vitamin K deficiency may suffer from internal bleeding, including brain hemorrhages, the condition can be life-threatening or result in severe functional disabilities. Despite this, recent data indicates that parental hesitation is growing.

According to the findings, the proportion of newborns not receiving the injection remains low in absolute terms but has increased over time. In 2006, 667 infants—or 0.66 percent of the total—did not receive the injection. By 2021, that figure had climbed to 1.50 percent of the cohort. The study notes that the proportion of children without a documented Vitamin K injection is higher for births occurring at home.

The study found that the risk of bleeding was twice as high for those who opted out of the injection, while the risk of intracranial hemorrhage—bleeding in or around the brain—was nearly three times higher. During the first month of a child’s life, the risk was found to be nearly four times higher compared to those who received the standard care.

Parental Concerns and Healthcare Skepticism

Researchers have identified several factors driving the trend, though the specific motivations of individual parents remain complex. Eleni Simatou notes that in other studies, parents have expressed concerns that the procedure may cause pain or that the injection may have side effects. Some parents believe that an increased intake of Vitamin K in the mother’s diet is sufficient, while others exhibit a general distrust of the healthcare system. She suggests that an increased skepticism toward vaccinations may also play a role, as studies show that parents who decline Vitamin K are often inclined to decline subsequent vaccinations.

Of the children in the study who did not receive the injection, ten percent received Vitamin K drops instead. However, these children also showed an increased risk of bleeding compared to those who received the injection. The drops must be administered once a week for 12 weeks, creating a risk that doses are missed or that parents stop the regimen prematurely. Furthermore, if an infant has undiagnosed liver or gastrointestinal issues, the body may struggle to absorb the vitamin, whereas the injection is administered only once at birth.

Future Strategies for Parental Communication

Eleni Simatou highlights the importance of continued information for both parents and healthcare staff regarding the benefits of Vitamin K and the risks of opting out. She notes that parents may not fully understand what they are declining. Regarding concerns about pain, she suggests that healthcare providers can discuss the use of breastfeeding during the injection to help soothe the infant.
