Transitioning to a Unified Digital Health Record

The era of managing multiple hospital-specific cards is coming to an end for residents in several Quebec regions.

For years, these physical cards served as a primary key for staff to locate paper-based medical files. Because each hospital managed its own archives, patients who frequented multiple facilities often carried a stack of different cards. The new system consolidates this information, allowing a single identity to grant medical personnel access to a patient’s digital dossier regardless of which participating site provides the care.

Retaining the Provincial Health Insurance Card

While the hospital card is being phased out, the provincial health insurance card remains mandatory. Patients must continue to present it during appointments and when receiving services, as its unique number now serves as the primary identifier within the DSN. Authorities advise that those residing in the affected regions may now safely destroy their old hospital cards to protect the personal information printed on them, provided they are certain the card originated from a facility where the DSN is currently active.

A critical caveat remains for travelers. The transition is not yet universal across the province. If a patient visits a facility in a region where the DSN has not been deployed—such as during a trip, while camping, or staying at a cottage—they should continue to carry that specific hospital card to avoid administrative delays.

Investment and Administrative Friction

For patients in the Mauricie and Centre-du-Québec regions, the shift represents a tangible reduction in paperwork. The success of this transition now hinges on the continued expansion of the DSN to other regions, which aims to eventually eliminate the need for region-specific cards entirely.
