The United States is experiencing a record-breaking outbreak of cyclosporiasis, a parasitic infection causing severe gastrointestinal distress. As of July 2026, thousands of cases have been reported, with Michigan alone identifying over 3,300. Officials warn that the actual count is significantly higher due to diagnostic challenges and underreporting.
Diagnostic Hurdles and Clinical Challenges
The current outbreak of Cyclospora cayetanensis has overwhelmed public health systems, particularly across the Midwest and Northeast. According to reporting by Emjreviews, the parasite presents a unique clinical challenge because it cannot be cultured in laboratories. This limitation forces medical providers to rely on specialized molecular assays or ultraviolet fluorescence microscopy, where the oocysts naturally autofluoresce.
Standard stool tests often fail to detect the infection because of low and intermittent shedding of the parasite. As a result, clinicians must maintain a high index of suspicion for patients presenting with prolonged, remitting, watery diarrhea and extreme fatigue. Once confirmed, the standard treatment is trimethoprim-sulfamethoxazole; however, managing patients with sulfa allergies remains difficult due to a lack of effective alternative therapies.
The Scale of the Infection and Transmission Risks
While official tallies have reached nearly 7,000 potential cases, experts suggest the true burden of the disease is likely much higher. Jeanne Marrazzo, CEO of the Infectious Diseases Society of America, noted that many individuals do not seek medical care, and standard diagnostic panels often omit testing for cyclosporiasis. The parasite is primarily transmitted through fresh, raw produce, including lettuce, herbs, and berries. Unlike other common foodborne pathogens, the oocysts are highly resistant to standard industrial sanitizers, including chlorine and bleach. Norman Beatty, an associate professor of medicine at the University of Florida, explained that the parasite’s structure allows it to lodge deep within the crevices of fruits and vegetables, making them difficult to clean effectively.

Precautionary Measures in the Food Supply Chain
In response to the rising case numbers and identified links to tainted lettuce, some commercial entities have implemented voluntary safeguards. For instance, Taco Bell confirmed it had voluntarily and temporarily removed limited ingredients at select restaurants as a precautionary measure.
These actions highlight the difficulty of securing the produce supply chain against a pathogen that survives traditional washing methods.

Public health authorities continue to urge patients to report symptoms to local health departments to assist in federal traceback investigations. Because the infection can have a debilitating, long-term impact on daily life, health officials emphasize the necessity of clinical vigilance. As Marrazzo noted, A lot of times with diarrhea, you have an episode and you feel better. With this, it just can kind of go on and on, and it really takes people out.
