Avoiding high blood pressure, diabetes, and smoking during middle age could buy people roughly 13 more dementia-free years, according to a study published in Neurology Open Access. Researchers followed more than 12,400 participants for about 30 years to track how vascular risk factors shape aging trajectories and overall brain health.
The link between cardiovascular health and brain longevity has long been a focal point for medical researchers who observe that what protects the heart generally protects the mind. Tracking Middle-Aged Risk Factors Across Thirty Years
To understand how heart-related health affects long-term neurological outcomes, researchers examined more than 12,400 middle-aged individuals whose average age was 56 years old, according to the Msn report. At the start of the study, none of the participants had dementia. The team measured three specific risk factors: whether participants had high blood pressure, had diabetes, or currently smoked.
Following the initial baseline measurements, researchers tracked the cohort for about 30 years. This longitudinal observation relied on periodic cognitive tests conducted via phone and during in-person visits, alongside medical records to determine who eventually developed dementia and who passed away without cognitive impairment.
Quantifying the Cost of Vascular Strain on Dementia-Free Survival
The results demonstrate a direct correlation between the number of vascular risk factors present at midlife and the reduction in dementia-free years. Participants who started with zero of the tracked risk factors managed to skirt dementia for 30.1 years. In stark contrast, individuals who presented with all three risk factors lived just 17.5 years without the condition.
Each additional risk factor—whether high blood pressure, diabetes, or smoking—was tied to about four fewer years of dementia-free survival, according to Coresh. Consequently, individuals with a single risk factor lived 26.3 years without dementia, while those managing two risk factors lived 21 years free of the condition.
Heather Whitson, director of the Duke Center for the Study of Aging and Human Development, who was not involved in the research, emphasized that avoiding both early non-dementia mortality and cognitive decline defines true dementia-free survival. Part of the drop in dementia-free years stems from individuals with heart-related risk factors dying sooner from non-dementia causes like heart attacks or strokes, while the remainder reflects individuals developing dementia earlier in life.
Biological Mechanisms Linking Vascular Damage to Brain Health
While the study was observational and cannot definitively prove that avoiding these risk factors directly caused a delay in dementia, health experts point to well-established biological drivers to explain the connection. Each tracked risk factor damages blood vessels, which impairs the brain in multiple ways.
Furthermore, vessel damage triggers an inflammatory reaction. Whitson noted that the body ramps up inflammation to repair damaged vessels, but this process often results in poor healing, plaque buildup, and chronic inflammation that further compromises neurological health. Coresh added that vascular disease may also enhance the accumulation of Alzheimer’s disease proteins like amyloid in the brain.
Demographic Variations and Study Limitations in Long-Term Tracking
The protective association between cardiovascular health and extended dementia-free survival held true across all demographic groups, though important variations emerged. Black participants consistently lived fewer years without dementia compared to their White counterparts, whereas women lived longer without dementia than men. These patterns suggest that early health interventions could yield particularly powerful benefits for certain populations.

The research design carried notable limitations. Because the study was observational, it establishes links rather than absolute causality. Additionally, researchers checked participants’ vascular risk factors at only one moment in time, leaving open the question of whether lifestyle changes made later in life—such as picking up or quitting smoking—might alter an individual’s dementia-free trajectory.
